1. Submit this form below with all the documents required.
2. The JY office will send you a unique link to your email to complete your profile on the UK Gov website that handles DBS. This link expires in 48 hours - please be prompt to complete this step. Otherwise this will cost us to send another application again.
3. You will receive the DBS certificate to your home address (you will fill this in on the Gov website) by mail.
4. Submit a picture of your DBS to complete DBS registration with Jesus Youth UK. (another form available in the safeguarding portal).
