Before submitting this form, you need the Program Master Sheet ready.
The purpose of a risk assessment is to identify potential risks or hazards that could cause harm to those attending an event. The assessment should evaluate all risks and hazards and ensure appropriate measures are implemented to mitigate, eliminate, or control them. Organisers must review and update these assessments to address any new issues that may have arisen.
If this is a repeat event, give it a unique name. e.g. "Exodus Weekend February 2025"
This is the Google Sheet URL to the Master Sheet for this event which includes the program list, attendees, volunteers and room allocation. This document would have been created and shared with you already once your region's safeguarding lead has registered this event.
If the location of the event changes or the frequency is more than a month, this form needs to be submitted before each event.
